Some T-shirts are incredibly thin and in our opinion unsuitable to add any embroidery onto. Often the white ones are even thinner than the colours and any amount of stabliser you use won't result in an acceptable finished garment.
I'm not sure what problems you are having, but symptoms like puckering, or holes appearing could all be down to the quality of the shirt rather than anything you are doing wrong.
Use a cutaway stabiliser (maybe even two layers), make sure your designs are not too dense, and finally, try not to stretch the shirt too much in the hoop.
